Did you ever get to the bottom of this?  I have one controller that requires two resets every time power is cycled.   Once reset it runs without a problem.  Other controllers are hit and miss on power reset and some are rock solid.

It was the Pic chip. It has operated perfectly for 3 years since RJ sent me a replacement chip.
