In your picture in the first post. One of the white chips is installed backwards.
Do you have any better pictures of the front and back of the board.

Here are the best pictures I was able to take this morning. In the pictures the regulators mounting is pretty ugly since I've just soldering them on the topside of the pad to make for easy removal/troubleshooting.
